Clinical Note Fields

Nookal provides user-friendly Templates to save time while writing Clinical Notes.

Below explains the different Field Types available and how to set them up. For more instructions on creating Clinical Notes Templates, click here.

Heading

  • Split a Clinical Template into different sections using Heading.
  • The Label is the heading.

Date / Time

  • Easily select a date and time to record information such as injury date, surgery date etc. It is possible to set default dates.
  • Add Help/Info text to assist in knowing what information is required in this field.

Field preview:

Chart

  • Draw and write on a Chart.
  • Select a default Chart that can be changed at the time of writing the note by selecting any available Charts uploaded in the account.
  • Add Help/Info text to assist in knowing what information is required in this field.

Text

  • Type text or use snippets and symbols to make writing the note quicker.
  • Default text can be set up that will remain in the note if unchanged.
  • Add Help/Info text to assist in knowing what information is required in this field.
  • Tick the Show Mirrored option to have 2 textboxes side by side (e.g. to record details for left/right side).

Multiple Choice

  • Select one option out of the list have created.
  • Add as many options as needed to choose from.
  • Default selection can be set up which will be in the note if unchanged.
  • Add Help/Info text to assist in knowing what information is required in this field.
  • Tick the Show Mirrored option to have 2 columns of choices side by side.

Preview field:

Checkboxes

  • Tick as many options as required.
  • Add as many options as required to choose from.
  • Defaults selections can be set up which will be in the note if unchanged.
  • Add Help/Info text to assist in knowing what information is required in this field.
  • Tick the Show Mirrored option to have 2 columns of choices side by side.

Field Preview:

Dropdown

  • Select one option from the dropdown list created.
  • Add as many options as needed to choose from.
  • Default selection can be set up which will be in the note if unchanged.
  • Add Help/Info text to assist in knowing what information is required in this field.
  • Tick the Show Mirrored option to have 2 dropdown lists side by side.

Field preview:

Number

  • Field restricted to numbers only.
  • It is possible to set a minimum and a maximum value to ensure an accurate value.
  • Set a default value if relevant.
  • Numbers can be set by using the arrows on the side of the field when writing clinical notes.
  • Add Help/Info text to assist in knowing what information is required in this field.
  • Tick the Show Mirrored option to have 2 number fields side by side.

Scale

  • Slide a button on the scale to set a number.
  • It is recommended to set minimum and maximum values.
  • Set a default value if relevant.

  • Tick the Show Mirrored option to have Scale fields side by side.
